We aim to be your complete “medical home” to provide your children exceptional medical care from a doctor and nurse you know and can trust. As a full service office, we are here for your children from the moment they are born until they are young adults. Contact our office to learn more.

School and Camp Forms

Please note that most forms require a routine physical examination within the preceding 6-12 months. Please fill out all requested information in the parent section. It takes approximately 5 working days to complete the form. There is a nominal charge for the completion of this paper work.

In Case of Emergency

Telephone calls after hours should be limited to emergencies and urgent medical problems only. If you have a problem that cannot wait until morning, you should call the after-hours phone number for your pediatrician. Your call may be answered via voicemail, which will provide information about contacting the doctor on call for the evening or weekend.

Appointments

Here at Meridian Pediatrics, our physicians, nurses, and staff remain here for you and your family during these unprecedented times and are taking additional measures to keep your family’s health our top priority. For additional information, please call our office.

Patients are seen by appointment only. Our office does not accept walk-in appointments. Regular office hours are Monday through Friday, 8:30 a.m. – 5:00 p.m.

We prefer to schedule appointments based upon individual needs and strive to not keep patients waiting. Please be prompt for your appointment. We will do our best to be on time, but occasionally emergencies and other unexpected events may cause us to fall behind. If we are more than 15 minutes behind we will try to explain the situation so that you can determine whether you would like to reschedule.

Telephone Calls

Doctors give medical advice during office hours. When you call, the medical receptionist will carefully transcribe your concerns in a note to your doctor. The doctor will review the note and instruct the receptionist or nurse to advise you accordingly. Note: we are now able to review photos per HIPAA-compliant email. Insurance and billing questions will be accepted between 9:00 am and 5:00 pm at (317) 844-4340.

Medical Home
What is a Medical Home?

A medical home is an approach to providing comprehensive primary care that facilitates partnership between patients, physicians, and families.

The American Academy of Pediatrics (AAP) believes that every child deserves a medical home, where care is accessible, continuous, comprehensive, patient- and family-centered, coordinated, compassionate, and culturally effective.

Over the past year the AAP has been at the forefront of numerous initiatives to promote the adoption and spread of the patient- and family-centered medical home.
