School and Camp Forms
Please note that most forms require a routine physical examination within the preceding 6-12 months. Please fill out all requested information in the parent section. It takes approximately 5 working days to complete the form. There is a nominal charge for the completion of this paper work.
In Case of Emergency
Telephone calls after hours should be limited to emergencies only. If you have a problem that cannot wait until morning, you should call the home of your pediatrician. Your call may be answered by a recorder, which will provide information about contacting the doctor on call for the evening. Click here for a list of our physicians.
If your doctor has an early morning phone time, you may call to discuss any type of problem or concern. Parents can use this time for voicing concerns prior to a scheduled well visit, gaining insight as to feeding, discipline, toilet training, daycare, etc. The doctors give medical advice during office hours. When you call, the medical receptionist will carefully transcribe your concerns in a note to your doctor. The doctor will review the note and instruct the receptionist or nurse to advise you accordingly. Insurance and billing questions will be accepted between 9:00 am and 5:00 pm at (317) 844-4340.
What is a Medical Home?
A medical home is an approach to providing comprehensive primary care that facilitates partnership between patients, physicians, and families.
The American Academy of Pediatrics (AAP) believes that every child deserves a medical home, where care is accessible, continuous, comprehensive, patient- and family-centered, coordinated, compassionate, and culturally effective.
Over the past year the AAP has been at the forefront of numerous initiatives to promote the adoption and spread of the patient- and family-centered medical home.
